Surgical Instructions
Our staff looks forward to meeting you and discussing your plastic surgery goals. We strive to keep our appointments on time, as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so that you can accelerate your check-in process. Please download, print and complete the forms and bring them with you to your appointment. If you are unable to download our patient forms, then it is recommended that you arrive to your appointment between 15-30 minutes early so that you can have ample time to fill out the appropriate forms.

Operations at HMH
For your safety and convenience, you will be evaluated by the Houston Methodist Hospital Anesthesia Preoperative Evaluation Clinic (APEC) before your surgery.
For your convenience, younger, healthier patients with few existing medical conditions may be able to be registered and screened over the phone. Patients with more serious existing medical conditions must be seen in APEC in advance. These patients may be asked to provide a letter of consultation from their internal medicine physician/cardiologist; this report can be faxed to APEC. For patients with cardiac stents or implanted defibrillators, an evaluation by the patient's cardiologist is typically required before surgery can proceed.
To determine which level of surgical clearance you will need please complete the online Anesthesia Pre-Op Evaluation questionnaire at least 7 days prior to your surgery date which can be found here
Once you have completed the questionnaire please call the clinic at 713-441-5035 to inquire if you will require further testing.
